When the full economic cost of permanent medical staffing is properly measured , locum doctors are not only defensible - they are often better value .

Much of the opposition to locums comes from commentators who:

Permanent doctors attract employer superannuation contributions (currently 11% , legislated to rise to 12% ). This alone represents a significant hidden cost.

Locum doctors:

Do not receive superannuation
Cover their own medical indemnity
Pay for income protection, public liability, and other insurances
Carry the commercial risk of downtime between contracts

Again, these are real costs - they are simply not borne by the health service .

What Does a Locum Agency Actually Cost?

Australian locum agencies typically charge 12–15% of the locum’s daily rate to deliver a complete end-to-end service, including:

National and international doctor sourcing
Credentialing and compliance (AHPRA, college requirements, references, police checks, immunisations)
Contracting and payroll administration
Travel and accommodation logistics
Ongoing workforce management and problem resolution

This margin funds entire recruitment, HR, compliance, and logistics functions .

Replicating this internally would require:

Dedicated recruitment teams
Compliance and credentialing officers
Payroll and contracting specialists
Travel coordinators
Legal and industrial relations infrastructure

When fully costed, public health systems cannot match private agency productivity or efficiency , particularly at scale.

Why Agencies Can Deliver This at 12–15%

Agencies achieve this efficiency only through:

Deep specialisation
Technology-enabled workflows
Automated compliance and credentialing systems
Virtual assistant (VAS) and offshore administrative support

Public systems, constrained by legacy IT, rigid procurement rules, and inflexible workforce structures, are not designed to operate at this level of output per dollar.

Locum Spend Goes Directly to Frontline Care - Not Bureaucratic Absorption

Another frequently ignored reality is where the money actually goes .

Every dollar spent on a locum doctor is directly invested in frontline clinical care . It pays for:

A qualified doctor delivering patient care

This makes locum expenditure:

In economic terms, locum spending has near-zero leakage . Funds are converted into clinical hours almost immediately.

Permanent staffing budgets, once absorbed into the system, become diffuse. Money is often redirected toward non-clinical functions that do not reduce waiting lists, increase throughput, or improve patient outcomes.

A Simple Test: Where Does the Patient Benefit Appear?

If a service invests an additional $1 million in locums:

More shifts are filled

If the same $1 million is absorbed into a health service budget:

Locums Are a Strategic Workforce Tool - Not a Failure

Locums are not evidence of poor planning. They are a strategic workforce lever that allows health services to:

Avoid unsafe staffing gaps
Manage leave, vacancies, and demand peaks
Protect permanent staff from burnout
Control long-term fixed costs

Used properly, locums reduce risk , stabilise services, and protect patient care.

Conclusion: Measure the Whole Cost - Then Decide

The claim that locums are “too expensive” rarely survives serious scrutiny.

When you properly account for:

Leave entitlements
Superannuation
Insurance and risk
Wage escalation
HR and compliance overhead
Bureaucratic leakage

Locum doctors are often equal or better value , with far lower long-term liability.

If governments and health executives are serious about efficiency, transparency, and patient outcomes, locums should not be viewed as a cost to be cut but as a high-value clinical investment that directs funding to the bedside, not the back office .

From Scotland to Brisbane: A GP’s Journey Through AHPRA’s Expedited Specialist Pathway

Every so often, a placement stands out, not because it was the easiest, but because it marked a turning point in how we support GPs moving to Australia. Dr Idle’s journey from Scotland to Brisbane is one of those stories.

When I first connected with him, the AHPRA Expedited Specialist Pathway was in the works but hadn’t officially started. He had initially submitted his RACGP Specialist Assessment, and then, the new pathway was announced, promising a much faster, cleaner route for UK GPs with CCT + MRCGP.

We moved quickly, transferring his RACGP application and switching him over to the ESP to speed up his application. He became one of the very first GPs to go through this new process.

And while the pathway itself was designed to be “expedited,” the reality of being first through the door was… interesting.

There were delays, long pauses, and moments where we clearly saw AHPRA navigating a new internal process while dealing with a high influx of applications. Nothing moved clean or in a way we had expected.

Because we corresponded directly with AHPRA daily, we were able to follow up, clarifying requirements, escalating where appropriate, and making sure nothing slipped through the cracks while the system caught up with itself.

Eventually, after what felt like a marathon for everyone involved, Dr Idle was officially registered, visa approved, and on a plane to Australia.

Today, he’s thriving in an exceptional practice, fully settled, and building exactly the future he hoped for when he first reached out from Scotland. It’s been almost two years now, and we’re still in touch, not just for professional updates but to support him as needed.

Our support continues long after a doctor starts their new role. Relocating your entire life to the other side of the world is a huge step, and whether it’s career progression, financial, schooling, community, or simply a friendly chat, we’re here for it.

And personally, even after twenty years in GP recruitment, it still gives me genuine joy when I finally meet a doctor in person after months of online correspondence and guiding them through the maze of registration, visas, logistics, and relocation. It’s such a privilege to help UK-trained GPs take the leap and build a new life and career in Australia. One of those GPs is Dr Bilikis, who we recently placed in a top medical practice at the stunning Blue Mountains, New South Wales.

I asked her to share her journey — from why she decided to move, to the challenges she faced, and what life is like now. Her reflections offer invaluable insights for other GPs considering the same path.

“I trained in the UK and worked there for two years before moving to Australia in May 2025.

I wanted to work in a less urban environment, somewhere with a patient demographic that felt more at peace with me. I’ve always loved mountain surroundings — even just the view travelling to and from work in Katoomba gives a calming effect.

The top reasons I considered Australia were:

To move away from a stressful work environment without needing to sit further GP exams.
To enjoy a warmer climate for most of the year.
To have more autonomy in my daily work schedule.”

“Of course, I had concerns:

Cultural diversity and whether I’d find a community I connected with.

Access to mosques and places of worship.
Take-home pay after tax.
Whether patients would like me based on my ethnicity and religion.”

Registration and Visa

“I went through the AHPRA’s Expedited Specialist Pathway.

It took me about six months to gather all the documents for conditional specialist registration with AHPRA and board approval.
Then another month and a half to complete the documents for my visa and secure a subclass 482 visa.”

“The Medical Centre was fantastic. They provided four weeks of temporary guest house accommodation and even gave me a supporting letter when I needed to rent a permanent place. They also helped with school options for my daughter, transport advice, and relocation allowance.

Background: What AHPRA Promised

AHPRA introduced the Expedited Specialist Pathway (ESP) on 21 October 2024, aimed at specialist international medical graduates (SIMGs) from jurisdictions with comparable training systems—initially focusing on General Practitioners (GPs), with anaesthesia, psychiatry, and obstetrics & gynaecology added by December 2024 ( ahpra.gov.au ).

It was marketed as a faster alternative to traditional specialist pathways, aiming for:

Application assessment in 4–6 weeks
Full registration and practice commencement within 4–6 months
Lower costs, less duplication, and a streamlined process

But has it delivered?

Where It’s Falling Short

1. Fewer Doctors Registered Than Expected

As of May 2025, just 127 SIMGs had been registered under the pathway—out of 251 applications received. This includes:

125 GPs
1 anaesthetist
1 psychiatrist

This implies that roughly 50% of applicants are still waiting for registration—a significant lag behind expectations. ( RACGP NewsGP )

2. Documentation & Verification Delays

While the promise was quick turnaround after submitting a "complete" application, in practice, many applications remain incomplete for weeks or months due to documentation issues.

One major hurdle is:

AHPRA requesting further evidence of training, particularly when the doctor's basic training was partially completed outside the UK.

This training may not be recorded on the General Medical Council (GMC) certificate, forcing doctors to go back and retrieve transcripts or evidence from countries where:

English is not the first language
Medical documentation standards differ
Institutions may be unresponsive or slow

As a result, applicants can face significant delays even before the official 4–6-week processing window begins.

3. Delays in Supervision Planning & Job Approval

Another common stumbling block is securing an approved supervised practice role. AHPRA requires a compliant job offer, nominated supervisor, and a formal supervision plan, all approved before registration is granted.

But in many regions, especially rural or public health settings, employers are unfamiliar with the process or slow to provide the required paperwork. This adds further delays, pushing registration well beyond the ideal 6-month window.

4. Concerns from Medical Colleges

The ESP has also drawn criticism from major Australian medical colleges:

RACGP, ANZCA, and others have voiced concerns about the lack of Fellowship requirements, warning that doctors may enter practice without comprehensive assessment of their skills or familiarity with Australian health systems.
Some have even called for the program to be paused, citing risks to patient safety and doctor support.

These concerns have led to confusion among SIMGs, many of whom are unsure whether to proceed via the new ESP or the traditional college pathway.

The Gold Coast turned it on again for this year’s RANZCP Congress. It was such a pleasure to be on the ground meeting the incredible doctors we work with- some familiar faces, some new-and reconnecting with the health services we proudly support throughout the year.

Tracy, James, and Linh were there representing the MediPeople team, and as in previous years, we were proud to sponsor the massage station. It’s become a bit of a tradition now, and it’s always a joy to offer a moment of calm and connection during a busy few days for our doctors and clients.

UK-based psychiatrists now have an exciting opportunity to fast-track their career in Australia through a new Expedited Specialist Pathway, designed to reduce barriers and help internationally trained specialists transition seamlessly into the Australian healthcare system.

This new pathway is significantly faster and more cost-effective than traditional routes, reducing the time to start practicing in Australia from 12 months to approximately 4–6 months.

state border developments. You will find an overview below with the requirements for doctors to enter each state. While this information will change over time, the details below will give you a good introduction to the current situation .

ACT

There are no longer any restrictions on interstate travel to the ACT.

NSW

There is currently no requirement to complete an entry declaration form when travelling to NSW. If you are allowed to travel to NSW from a location that appears in the COVID-19 concerns notice, when you arrive in NSW you may be required to:

follow the interstate traveller guidelines if you have been in an

area of concern or

place of high concern

follow the COVID-19 rules and restrictions that apply in NSW

get a COVID-19 test if required to do so.

You must Complete a Border Entry Form before arriving in the NT

You need to be fully vaccinated

A pre-arrival COVID-19 PCR test is not required for interstate travellers entering the Northern Territory.

You must do a Rapid Antigen Test within two hours after arriving in the NT.

Have a Rapid Antigen Test on Day 3 after arriving in the NT.

Have a Rapid Antigen Test on Day 6 after arriving in the NT.

As proof, a photo must be taken of each of the test results, the packaging displaying the batch number and a document identifying yourself, this must be retained and provided on request. You must submit an online declaration stating that you completed the 1st test within 2 days of arrival and additional tests no later than Day 8 after arriving in the NT.

QLD

You can enter Queensland by road, air, rail, or sea.

You must be fully vaccinated

Domestic travellers entering Queensland are required to complete a declaration before entering Queensland.

Complete the Border Pass if you are a Border Zone or Queensland resident undertaking border zone travel for an essential purpose or fully vaccinated and travelling within the non-restricted Border Zone.

You need to have a negative COVID-19 test (PCR or Rapid Antigen) taken within the 72 hours prior to arrival into Queensland. Both test and result.

Interstate travellers are no longer required to complete an EntryCheck SA application. You will need to comply with the QR code check-in requirements for each venue.

TAS

All travellers must register through the TAS e-Travel system.

You must be fully vaccinated or have an exemption.

You must have evidence of your vaccination certificate and – if required – declare a negative result from your COVID‑19 test taken before departure.

If you have been in a high-risk area in the 14 days before you intend to arrive in Tasmania you must return a negative result from a COVID-19 rapid antigen test taken in the 24 hours prior to departure, or a PCR test 72 hours prior to departure, to Tasmania.

VIC

No permit required to enter Victoria from interstate unless you have been overseas in the past 14 days.

No domestic testing requirements for interstate arrivals.

You do not require a permit if you have completed hotel quarantine interstate and wish to enter Victoria.

Border restrictions are set to change from 05 Feb 2022. The state intends to ease interstate travel restrictions then. The current requirements are:

A G2G Pass is mandatory to enter WA

You must be fully vaccinated

14 day quarantine is required when entering from medium, high and extreme risk states (every Australian state currently rate from medium to extreme risk)

Dr Alex’s Experience as a Locum GP in Rural NSW

I recently undertook a locum Rural GP VMO placement arranged by Chris at Lightning Ridge, Western NSW, for the VMO position.

Lightning Ridge is the wild-west of NSW, with an eclectic case mix of patients in an area renowned for opal mining, sunsets and a simple way of living.

What is Locum Rural GP VMO work like?

The role consisted of working in the local GP clinic, with on-call duties at the hospital and management of patients at the emergency department .

I thought this to be daunting on my first day but the close proximity of clinic and hospital, fantastic practice manager and supportive team at Lightning Ridge, in addition to the Virtual Rural Generalist Team providing clinical support for difficult clinical presentations, made the work as safe and enjoyable as possible.

Yes, whilst there was an on-call component, the system works that VMO's are only required to return to see patients in the ED if they are a category 1 or 2 triage, or complex 3 requiring onsite input, for the 'after-hours' component.

The staff at LR were kind, helpful and worked amazingly as a team to overcome many of the challenges faced when caring for unwell patients in a resource scarce setting.

I felt genuinely included in the team and quickly brought up to speed in the orientation phase, I rate the clinical staff at LR as one of the real reasons the experience was so enjoyable.

With the exception of one emergency presentation of a neonatal resuscitation requiring NETS transfer, which was a heart-mouth-experience to say the least, I found the workload and on-call to be manageable, incredibly satisfying and most nights I didn't have any on-call presentations past 10pm, but can only speak of my experience.

Accommodation was nearby to the hospital, clean and tidy. The drive to the location was stunning and covered many changing landscapes of rural and remote NSW, and was a memorable experience unto itself.

Would you do it again?

Overall, I thoroughly enjoyed my time at Lightning Ridge in the GP VMO role, based on the wide rage of exciting medical presentation in the acute and chronic settings, fantastic local team, and opportunity to deliver high quality healthcare to a population that battles with many challenges of being remote and far from the type of health resources many of us take for granted.

I am extremely grateful to Chris who arranged and negotiated the position for me, and was invaluable throughout my time in answering my questions and supporting my experience in ensuring a safe time on the placement.

When I decided I wanted to extend my placement a further week, Chris seamlessly handled the negotiation of the time, safe rostering and great remuneration rate, so I could focus on delivering quality work and enjoying my clinical experience.

I am excited to continue working with Chris for my next locum adventure, and will make Lighting Ridge a regular stop on my itinerary.

A Day in the Life of a Doctor at a Pop-up Covid Community Testing Station

We dug a little deeper to see what Dr Anna’s day at a pop-up COVID community testing station looks like:

Where are you based?

Community testing means I am often at different COVID testing sites in car parks across Melbourne Metro including MSAC, Darebin etc.

How does your shift start?

The shift often starts in the morning with placing our orders for lunch and snacks (yes lunch and snacks are provided). We then have a handover in the morning and start reviewing patients/cars that come through the drive-through COVID testing centre.

What does the role involve?

The role involves scanning a barcode which the nursing staff have pre-filled, then asking the patients about any covid symptoms. We essentially have to confirm the details that were previously provided and submit the test under our provider number.

How many on the team you work with per COVID testing site?

Often there are around 15-20 people per COVID testing centre I work with.

What support do you have?

There is always a coordinator in charge that you can contact. This is not really like the hospital where you have a senior medical doctor, you just have a support worker to assist with recent guideline changes and answer any questions you might have.

What is wearing full PPE like?

Full PPE is very easy, just a gown, mask, goggles, and gloves. You get used to wearing it all day.

Which PPE items do you have available?

You have all the PPE items available and will wear it the whole shift.

Do you feel that your job is risky?

In the community I do not feel this job is risky given low transmission now, and given the outside dispersion of particles.

What do you enjoy about doing the COVID testing shifts?

This is a relatively easy, social job. It is nice to work with a wide range of people and not have a high stress load for the job.

How satisfied are you with this work?

This is great work, easy to complete, nil stress, and enjoyable.

Have you dealt with any positive patients so far?

Not that I am aware of. On the days that I test, I often check the next day to see if we have had positive cases. So far on the shifts that I have worked I do not believe I have come across any.

Any advice for other doctors thinking of doing COVID testing work?

It is a great job to do in your free time. Just ensure you wear the PPE correctly.

A Day in the Life of a Doctor Working in a Covid Hotel

We dug a little deeper to see what our doctor David’s day looks like:

Where are you based?

I’m working at a hotel which is very central to Melbourne and ridiculously close to home for me.

How does your COVID shift start?

I walk in and greet the ADF staff at the door who take my temperature, ask me about my exposure to hotspots, and check my identity via my license. My name gets entered in an entry log with time of entry.

I then walk into our team’s designated covid testing area and say hi to my regular team of nurses.

What does the role involve?

The doctor’s role is largely checking ipad entries from the triage nurses, answering the occasional clinical question, and being flexible enough to help the nurses with any workflow problems.

How many on the team you work with?

In my team there are usually 4 nurses and one pathology collection person.

What is wearing full PPE like?

I haven’t found wearing PPE all that onerous. On hot days your hands can get a bit sweaty but being in an air-conditioned hotel means this is rarely an issue.

Which COVID PPE items do you have available?

Face masks, N95, multiple glove sizes, gowns and visors.

Do you feel that your job is risky?

I don’t feel that this job is risky at all. We’re testing hotel staff who are asymptomatic. If any staff are symptomatic they are advised to stay at home. If they become symptomatic at work they are swabbed in a separate area and sent home until symptoms resolve and they return a negative covid test.

What do you enjoy about doing COVID shifts?

It’s genuinely fun working with largely the same group of people day to day in an environment that is completely stress free. The hotel staff get to know you very well as well. This makes for a very social day, every single day. It’s terrific fun.

How satisfied are you with your COVID work?

I love this work because it’s very social as I’ve said, but you can also get studying and reading done during down time.

Have you dealt with any positive patients so far?

I’ve been working in this job for several months and I have not encountered one single positive patient.

Advice for other doctors thinking of doing this?

This job is very easy, very social and pays really well. You’ll seriously love it!

Has the locum life been as good as you thought it would be?

Locum life has been very interesting! The main reasons that I decided to locum was to travel, work in a new variety of different places/hospitals and to save money for my planned three-month trip to South America. In the first few months there was a lot of interstate travel which I found to be very exciting! The range of experience obtained was excellent, too. Locuming presents a fantastic opportunity to develop new friendships and to network with other doctors.

Since COVID19 obviously my travel has dramatically reduced, and I have chosen to stay in a long-term locum position. I have greatly enjoyed this role; however, I certainly miss the travel!

Has locuming helped you to achieve what you wanted to?

Yes, I have had very positive experiences from my time locuming across Australia. Since I have started, I have grown substantially both professionally and personally. It has been an amazing experience to be able to work in a diverse range of places with wonderful people – being a doctor in Mount Isa is an incredibly different experience to being a doctor in a tertiary city hospital. I am certain that my skills have improved through locuming. Also, it is very financially lucrative.

Any standout locations you have worked at?

I have enjoyed all of the places that I have worked at. My most favourite would be Redcliffe Hospital (Medicine), Canberra Hospital (ED) and Ballarat (ED). These are the three places that I have spent the most time at – I was able to settle in, meet new people and become fairly accustomed to the unique processes of those departments. Certainly, as a locum, being able to establish a vibrant social life wherever you go will make the experience much more fun and interesting. I have found my current hospital to be a great place to work in, and very supportive.

What tips do you have for doctors considering the locum lifestyle?

I would strongly encourage anyone to locum, even if a short period of time. It’s a great way to take a break, travel and trial new places to work. I have enjoyed my most recent long-term locum engagement so much that I am now employed by the hospital and will be taking up a training position here next year – so I have officially “relocated” to a new city thanks to a locum job. This was certainly not planned when I first started the contract.

Locuming forces you out of your personal and professional comfort zone, which although nerve-wracking at times, is a very important and valuable experience.

How has COVID19 affected locum work?

Prior to COVID19 I travelled very, very frequently. My longest contract was a month and my shortest was two days. From mid-March Covid-19 hit, and travel was not recommended – thankfully at this time there were multiple locum job offers from various hospitals interstate wanting to hire doctors for 3-6 months.

I was meant to fly to South America in mid-March, so when this was cancelled my schedule became very vacant! I was lucky enough to be offered a four-month contract in my preferred speciality (Medicine). I chose not to fly to my locum placement due to safety concerns and I also strongly suspected the flights would be cancelled. Instead, I drove twenty-one hours, – with a great Spotify playlist – to the locum job! (Social distancing style).

What has your experience with Medipeople been like?

I have very much enjoyed working with Medipeople ! Justine my recruiter, has been extremely supportive and incredibly dedicated to keeping me very busy – which has been greatly appreciated! Occasionally as a locum doctor there are small gaps in-between contracts that can be very hard to fill – luckily, Justine has been very successful in minimising these.

Australia on Track for Doctor Shortages

Australia is heading for an acute doctor shortage in the coming decade, especially of full-time GPs. This trend emerges as fewer medical graduates enter the market and demand for health services rises, new research from Deloitte Access Economics shows.

By 2030, researchers project a shortfall of 9,298 full-time GPs – 24.7% of the GP workforce – according a new report released. Recent planning has failed to take into account the increasing trend for GP’s to work part-time and the growing demand for GP’s in subspecialties such as skin and cosmetic medicine.

The GP deficit is set to be most extreme in urban areas, with a shortfall of 7,535 full-time GPs (31.7%) by 2030, while regional areas can expect a deficit of 1,763 (12%).

The report recommends removing the limit on overseas-trained doctors working in urban areas to quickly boost labour supply. Recent changes to GP recruitment have made it harder for overseas trained doctors to work in Australia. Changes to District of Workforce Shortage has left many practices completely unable to find fully qualified and unrestricted GP’s. In addition tightened visa restrictions have increased the cost burden for practices who have no choice but to recruit a GP from overseas.

Implementation of the Stronger Rural Health Strategy (SHRS), to restrict overseas-trained doctors practising in urban areas will exacerbate the underlying supply shortfall in these areas, it says.

Over the medium-to-long term, the report suggests increasing registrar enrolments, particularly in urban areas, “will assist with delivering a better geographical alignment of the GP workforce to meet demand”. This may serve to curb the negative effects of the doctor shortage Australia is likely to experience in the next decade.

What It Means for Hospitals and Medical Practitioners

Subclass 482 / tss visa changes

The federal government’s latest round of changes to the skills sponsorship 482 visa has a major impact on hospitals and medical practices who need to sponsor doctors.

The biggest impact other than the delay/confusion created by the LMT is the Skilling Australians Fund (SAF) levy which creates a huge cost burden to the client.

It makes sense that Hospitals and practices think outside the box and not necessarily nominate visa applicants for the full 4 years where the medical practitioner could obtain PR shortly after arriving. 2 years would be good for Medical Practitioners in this scenario. Medical Practitioners over the age of 45 should apply for 4 year visas to enable them to take advantage of the Age Waiver through the Employer Nomination Scheme (ENS). This will come into effect after the 3-year anniversary of employment with an employer meeting one of the following scenarios:

they are nominated as an academic (university lecturer or faculty head at Academic Level B, C D or E) by a university in Australia
they are nominated as a scientist, researcher or technical (scientific) specialist at ANZSCO skill level 1 or 2 by an Australian scientific government agency
as a subclass 457 or TSS visa holder, they have been working in the nominated occupation for the last 3 years and your earnings were at least as much as the Fair Work High Income Threshold in each year over that period
they are a medical practitioner who has worked in the nominated occupation for at least 3 years as a 457 or TSS visa holder. For two of those years they were employed in regional Australia. The nominated position is in regional Australia

Medical Practitioners who applied for or held a 457 visa prior to the 18th April 2017 (and the visa was subsequently granted) may also apply if they are over 45 years of age but under 50.

Labour market testing

From 12 August 2018, The Department of Home Affairs made changes to Labour Market Testing (LMT) requirements which applies to any 457 or TSS nomination transfer or application. Previously hospitals have not had to complete LMT The added burden will increase delays for international medical recruitment and increase the burden on medical workforce teams.

A summary of the changes is as below:

The nominated position MUST be advertised in Australia
At least TWO job advertisements were published:
On a national recruitment website (Note: a general classifieds website is not an acceptable method)
In national print media
The Job Advertisements MUST have been posted within the 4-month period immediately prior to lodgement of the nomination application
The Job Advertisement MUST be in English and included ALL the following information: – The title or a description of the position
The name of the approved sponsor or recruitment agency being used by the sponsor
The skills or experience required for the position
The annual earnings for the position
The advertised position should be full time
If the job advertisement is published on a website, it MUST remain live for at least 28 calendar days.
If the advertisement is published in print media or on radio, it is expected that applications remained open or expressions of interest were accepted for at least 28 calendar days.
If there is any fee for advertising of the position you MUST provide a copy of the paid invoice showing a nil balance

If you have not already undertaken Labour Market Testing in line with the above requirements, this would be a good time to start.
Also, if the job advertisements previously posted do not meet the above requirements, new job advertisements will need to be reposted ASAP, in line with the above requirements.

The 482 visa nomination application cannot be lodged until two job advertisements, meeting the above requirements, have been posted and remained live for at least 28 calendar days.

What is the skilling australians fund (saf)?

In 2018, the Department of Home Affairs introduced a new levy applicable for businesses looking to hire overseas workers. The change is part of the government’s initiative to invest in training and upskilling of Australians and will prioritise funds for apprenticeships, traineeships, pre-apprenticeships and higher level apprenticeships, for occupations in high demand, areas with future growth potential as well as rural and regional Australia

All nominations lodged on or after 12 August 2018 must pay the SAF training levy for all subclass 482 visa applications.

Do we need to continue to meet the training benchmarks for 482 visa holders now that the saf is in effect?

Housing (3 Bedroom)

The new Skilling Australians Fund replaced the former training benchmark requirements, where businesses were required to spend at least 1% of overall payroll on training Australian employees or were required to make a contribution of 2% of payroll to a Training Industry Fund.

If you are an approved standard business sponsor, you are required to have met your obligations up until 12 August 2018. Your business will not be obliged to continue to spend any amount on training Australian employees from 12 August 2018.

Which visa subclasses does the saf apply?

The new Skilling Australians Fund (SAF) will apply for all new TSS visas (subclass 482) and permanent visa nominations (subclasses 186 and 187).

How is saf calculated for 482 and 186/187 visa nominations?

For Permanent Residency nominations (subclass 186 and 187), SAF is calculated based on the nominating company’s turnover.

Businesses with turnover less than $10M – $3,000 AUD
Businesses with turnover more than $10M – $5,000 AUD

For TSS (Subclass 482) visa nominations, SAF is calculated based on the nominating company’s turnover as well as the number of years you will be nominating the overseas worker.

Businesses with turnover less than $10M – $1,200 AUD per year
Businesses with turnover more than $10M – $1,800 per year

The levy is charged on a yearly basis. Any part of a year will be considered one when calculating the levy.

Your salary will be based on how many hours per week and for how many years you have been practicing. Your practice time counts from the date you completed your specialist psychiatry training. With every year of full-time work you have completed, you will move one step higher on the salary scale. In your first year after achieving your specialist/consultant qualification you will be on step 1. The next year you will be on step 2 of the salary ladder, and so on.

Additional benefits (such as superannuation and compensation for after-hours or on-call work) will be paid on top of the rates listed in the chart below. These benefits range from around $15,000 per year to up to $50,000 per year for the top steps. In April 2019 the remuneration for each step will increase as displayed in the chart. All psychiatrists who have been on step 14 for a year or more will progress to the new step 15.

457 Visa Changes Are Great News for Medical Doctors

The Department of Home Affairs has recently issued clarifications about professionals already on 457 visas who want to apply for permanent residency. There is good news! Medical doctors who have applied for a subclass 457 visa on or before 18 April 2017 will be eligible for permanent residency. They can apply under the Temporary Residence Transition stream. The provisions for doing so have remained largely unchanged:

Occupations: The eligible occupations remain the same as long as the nominee continues to work in the same position. And for the same employer (as of the Subclass 457 visa approval).
Age: The age limit will remain less than 50 years old.
Work experience: Work experience requirement will remain the same. Applicants will need to have worked for their sponsoring employers on a Subclass 457 visa for two years prior to making the application.
Duration and changing jobs: The visa is valid for up to 4 years. It enables the doctor to work for the sponsoring medical practice or hospital. In some circumstances it even allows the doctor to work in other medical practices.

The next set of reforms, impacting those who applied for their 457 visas on or after 19 April, are largely statutory. The Skilled Occupations List (SOL), which contains medical occupations, was renamed the Medium and Long-term Strategic Skills List (MLTSS). This list contains occupations which are of high value to the Australian economy, and includes doctors.

Upcoming 457 visa changes

Changes in July added a language requirement. An IELTS or equivalent test needs to completed scoring at least 5 in each component. The age requirement remains below 50 at the time of application to the Temporary Residence Transition stream.

From December the Department will collect Tax File Numbers for these visa holders. Data will be matched with the Australian Tax Office’s records to ensure that visa holders are paid their nominated salary.

The real changes, however, will happen from March 2018. These include:

Training levy: Employers nominating a worker for an Employer Nomination Scheme (ENS) or Regional Sponsored Migration Scheme (RSMS) visa must pay a contribution to the Skilling Australians Fund. The fee will be $3000 for companies with annual turnover of less than $10 million or $5000 for all others.
Occupation lists: Only jobs on the Medium and Long-Term Strategic Skills List (MLTSSL) will be available to foreign employees in the Direct Entry stream for both the ENS and RSMS categories.
Salary requirements: Employers will be required to pay the Australian market salary rate. They also need to meet the Temporary Skilled Migration Income Threshold ($53,900 as of 12 April 2016).
Residency period : The transition period to permanent residency will be three years instead of the current two.
Minimum experience: Foreign workers must prove they have at least three years of work experience relevant to the particular occupation.
Age limit : The age limit for applicants will be 45 years instead of 50 years at the time of application.

Conclusion

The big changes in 457 Visa rules impact those occupations that feature in the less favorable Short Term Skilled Occupation List. The biggest disadvantage being the visa does not provide a path to permanent residency. Doctors, however, feature in the preferred MTSSL list where they not only get four-year visa (indefinitely renewable). They also have the option of applying for permanent residency.

I am currently working as a psychiatry registrar in Townsville. I am currently posted to the Rural, remote and indigenous team. I am supporting various different teams across multiple locations with psychiatric assessment and treatment .

What is interesting about the role?

I get to travel a fair bit – I’m flown regularly to Palm Island to support the team there. The island has a fascinating living history and a unique challenges. It is like no other psychiatric experience I have had in the UK.

We often need to use telepsychiatry which I was at first skeptical about but have now embraced. I have had to learn to work with a new level of geographical barriers.

As a UK doctor moving to Australia, I have found the Australian culture to be surprisingly different to UK which in itself has been enlightening.

I have been able to use my British training experiences to try and improve psychiatry training generally and to staff in remote locations .

Importantly, I have felt really valued, respected and rewarded for my work here. I have quickly been recognized as someone who can help develop the service and have been given the opportunities to do so

How does the role benefit your training?

I have found it immensely beneficial to work in another system. I can compare and contrast differences in service. I can also develop my own leadership skills and systemic understanding. Working in another country has widened my experience and perspective of mental health care.

“I would definitely recommend working in Australia.”

I am working in Townsville located in tropical North Queensland. I had not heard about the city before researching jobs in Australia. It is a beautiful, hot and permanently sunny part of the world. It has been a great place to live with my young family. We swim in our pool on most days and have plenty to do. The city itself is has most things that one needs without being a metropolis. There is no traffic and the place is calm and quiet mostly. There are endless stretches of sandy beaches. I could not ask for more. I do not feel cut off from the rest of the world but feel far enough away for it to be a unique experience.

How did you find the role?

The relocation process to Australia is fairly complex ( AMC – AHPRA – College – Visas ). Agencies aim to make this process quicker and easier for doctors. Would you agree with this?

I was recommended Medipeople by a colleague. They asked what I was looking for and were able to advise on what was available. They were attentive and responsive to emails.”

It can be complicated and I would allow yourself at least 6 months to prepare for the trip. I came to Australia for an out of program experience year (OOPE). I required permission from the deanery and college before arranging the job. Often the college and the deanery need details about the Job before they can approve and so coordinating these and getting role descriptions etc may require the help of an agency like medipeople. The agency guided me through the application process and what was required. It can cost a fair amount of money to arrange a medical, VISAs and AHPRA etc. Many recruitment places will offer reimbursements for this.

As a UK doctor moving to Australia, arriving in Australia will require some setup costs also. I needed to get a car fairly quickly and to arrange accommodation. In total I spent more than the $5000 reimbursement for moving fees. The salary is considerably higher in Australia than the UK and so that deficit quickly normalizes. Many places will offer support with the initial move like temporary accommodation and a rental car etc. I managed to make the move with my wife and two children under 3 years old. It was a lot of work but I am so pleased we did it.

Since earnings will depend on gross income generated, it is difficult to put a cap on the amount you will possibly earn practicing in Australia. In a busy practice, daily billings can reach $2000 per day within just a few months of practice..

Flexible Billing can improve earnings

GPs in Australia can pretty much control the way they bill and the number of patients they see. Income depends on the number of patients seen and how they are billed. Bulk billing allows the practice to charge the patient a scheduled fee, based on consult time and extra if any procedures are involved and then claim payments directly from Medicare. GPs can also choose to private bill patients (cash) for services while the patient recovers a minimum consult charge from Medicare. Both methods can be combined into a mixed billing system which can be a great way to improve earnings and ensure you are one of the highest paid doctors. Earnings in Britain and Ireland

Even though GPs in Britain can increase the number of patients and expand services to increase income, earnings remain relatively fixed. Private medical work is usually limited to peripheral services. Another major issue is that there is no funding available for individual consultations, resulting in a lack of motivation for a GP to increase the number of appointments. The case in Ireland is also similar as the Health Service Executive (HSE) has stopped paying Non-consultant hospital doctors (NCHDs) for unscheduled overtime hours in some hospitals across Ireland.

It is a different situation in Australia when the sources of earning are considered. By expanding services and increasing the number of patients seen, income will increase. This increased income means there is motivation for GPs to work longer hours and see more patients which all leads to the highest paid doctors being in Australia.

Overtime pays well in Australia

Hospital Doctors in Australia are paid overtime when they work more than their 38-hour week, including weekends, public holidays and night shifts. This allows them to earn as much as double the standard earning rate. In practice, salaries can become 15-25% higher with overtime and on-call hours. As a contractor, ABN/ACN holder, you can pay as little as 30% tax on gross earnings, less if you have a good accountant. 1. Documentation

Take original or certified documents, which you may need before commencing your new role.
Ensure you have 100 points of ID with you.
Check your reporting instructions closely to see. Is the credentialing process complete?

2. Working with Children’s Check

The colleges’ reasoning for this is that Area of Need Psychiatrists have less support and need to be Substantially Comparable to be effective in this environment. Many people believe that this change was made to simply restrict the number of Specialist International Medical Graduates (SIMG) from entering the system.

The college assessment process in Australia takes 3-6 months to obtain an outcome. It compromises of a paper based application and interview. Most applicants don’t apply to the college until they have been offered a post as the application costs several thousand dollars. In addition to the college assessment, applications need to be made to the Australian Medical Council and Medical Board of Australia. While Australia does offer some of the best best jobs for Psychiatrists in the Southern Hemisphere, a Psychiatrist moving to Australia would be lucky to complete the whole process in under six months.

New Zealand Pathway Options

Psychiatrists moving to New Zealand can obtain registration with the Medical Council of New Zealand through two pathways.

The Vocational pathway is the standard pathway and involves an assessment of intentional psychiatry training in a similar way the RANZP assesses applications in Australia.
Special Purpose Locum Tenens pathway is open to Psychiatrists who trained in the UK, Canada, USA, UK and Ireland. This option does not involve a college assessment and doctors can complete it in under four weeks. There is a 12 month practice restriction on this type of registration but it allows you to move to New Zealand start working and complete the vocational registration from within New Zealand.

From a regulatory point of view the New Zealand pathway is much quicker, smarter and easier. This alone is often enough to make Psychiatrists move to the Land of the Long White Cloud .

FAMILY

Psychiatrists relocating with family members will have to consider the occupation of their partner and schooling for their children. In Australia schooling is not free for non-residents even in local government schools. Certain occupations may not be available outside of the major city or even in that country. Recent changes to visa pathways in Australia have also added further confusion – especially concerning spouse visas, English language assessments and the pathway to permanent immigration. People may also need to consider what the immigration policies are for parents/grandparents should they decide to stay on.

1. HAVE A SPECIAL INTEREST

GP’s who have a special interest such as aged care, mental health , obstetrics, etc. and develop their skills in this area, have a greater chance to build a patient base fast. Qualify this special interest as your subspecialty and you will be the go-to GP in the practice. The practice will channel these patients your way, and your colleagues will ask your opinion and refer patients to you. This is an excellent way to build a GP patient base and a great way to increase job satisfaction.

2. CONTACT YOUR PATIENTS

You deal with all aspects of your patient’s wellbeing so you’re their trusted health adviser. If you are medical practitioner thinking of moving to Australia you may be concerned how the 457 visa changes impact doctors and your future – Can I still get a visa? Can I move permanently? Please read onto see how the changes could impact you.

On 18 April 2017, the Federal Government announced plans to scrap the 457-visa program and replace it with a new TSS (Temporary Skill Shortage) visa.

Looking at the information supplied, Medipeople have assessed how this will affect Medical Practitioners

Medical Practitioner, General Practitioners and specialist medical doctors are currently listed on the Skilled Occupation List (SOL). The Skilled Occupations List (SOL) will be renamed the new Medium and Long-term Strategic Skills List (MLTSSL). This list will contain; occupations assessed as high value to the Australian economy, and occupations aligned to the Government’s long term training and workforce strategies. Currently a combined list on the department’s website clearly shows that medical staff and other healthcare workers remain on the list.

Occupations on the MLTSSL will continue to be issued for a maximum duration of four years. The Department of Education and Training’s is currently running 2017-18 SOL review and based on outcomes from this, the MLTSSL will be revised .

Eligibility criteria for all visas:

Work experience: at least two years’ relevant work experience.
Labour market testing (LMT): LMT will be mandatory, unless an international obligation applies. We will wait to see what requirements are made of medical practices.
Minimum market salary rate: Employers must pay the Australian market salary rate and meet the Temporary Skilled Migration Income Threshold requirements. No change here for Medical Doctors.
Character: Mandatory penal clearance certificates to be provided. Medics are already required to complete an International Criminal History Check.
Workforce: A non-discriminatory workforce test to ensure employers are not actively discriminating against Australian workers. No one really knows what this will involve.
Training requirement: a strengthened training requirement for employers to contribute towards training Australian workers. This may put off some smaller medical practices as the burden on small business increases.
English language requirements: a requirement of a minimum of IELTS 5 (or equivalent test) in each test component. Medics are already assessed by the medical board of Australia for English Language. However, a number are exempt. Will these exemptions carry over to visa applications?

What does this mean?

Although some of the details are still unclear, on the surface the 457 visa changes do not impact greatly on the current pathway for medical practitioners. The new system is going to be more complex than the current system with various short and medium migration streams. Will there be a faster or quicker stream for doctors moving to rural areas? Increased demands on the sponsor, like contributing to training Australians, may prove off-putting for businesses looking to recruit doctors from overseas. Once the MLTSSL is reviewed again, there will be pressure from medical colleges to have their profession removed from the list entirely. No matter what the exact changes will be, Australia will likely remain an attractive option – especially for UK doctors.

Increased application fees

The major changes to the skilled migration program will no doubt lead to an increase in application fees. Greater pressure on the visa processing department will also lead to even longer processing times at greater costs.

Australian Doctors Jobs in the UK

Brexit means big changes for Britain but not all of them are bad. Opportunities are going to arise for doctors from Commonwealth countries to fill the talent gap left by the exit from the European Union (EU), and none are placed better than highly trained affable Aussie doctors. Brexit is good news for Australian doctors jobs .

When I started in medical recruitment in 2002 for a boutique firm in West London things were very different. Not only were CV’s sent by fax machine, the nationality of the doctors we were recruiting was different too. The majority were from Australia and South Africa and armed with full registration from the General Medical Council (GMC) they were ready to go. Back then doctors from certain commonwealth countries were granted this full registration by the GMC without sitting any exams. These doctors were loved by our client hospitals across the UK. “Send me that friendly Aussie chap again,” they would say to us. Australian doctors are certainly well thought of in the National Health Service (NHS) and regarded as exceptionally well trained, hardworking and easy to get along with. On top of this their general registration status allowed them to work in non-training positions (Staff Grade) which was critical to making sure the health service did not completely fall apart.

What were the changes that impacted on Australian doctors jobs?

In 2005, an EC Directive disbanded the UK’s arrangements to favourable commonwealth countries like Australia and firmly confirmed the UK’s position within the EU. The recognition of professional qualifications laid down in (Directive 2005/36/EC) enabled the free movement of EU professionals such as doctors or architects within the Union. The new laws were designed to promote free movement of labour amongst the member states so EC nationals were given full registration with the GMC. Almost overnight medical recruitment companies were opening offices all over Europe to attract talent for the NHS.

What happened to the popular Australian doctors?

Australians were suddenly categorised as non-EU International Medical Graduates, and to obtain registration with the GMC Australian doctors were asked to pass The Professional and Linguistic Assessments Board (PLAB) exam. Australian doctors would then be asked to prove they could speak English and pass a clinical exam before being granted registration. In addition, changes to immigration laws made it much harder for Australians to obtain a visa to work in the UK. It’s not that it was impossible to make the move to the UK but changes to the laws made it more expensive, uncertain and less flexible. Those who just wanted to “give it a go’’ no longer bothered. Unless they were recognised specialists, the UK was telling Australian doctors not to bother because they were now part of the EU. The Australian doctors jobs had dried up.

Brexit impacts on medical recruitment

Jeremy Hunt has stated that the NHS in England will be “self-sufficient” in doctors after Britain leaves the European Union. He has set out a package of measures aimed at reducing its reliance on foreign-trained medics.

In a recent speech Hunt stressed that foreign-trained doctors do a “fantastic job”, and said that “we want EU nationals who are already here to be able to stay post-Brexit” before adding: “Is it right to import doctors from poorer countries that need them while turning away bright home graduates desperate to study medicine?”

The government plans for the NHS to reach its target in 2025. “Of course it will take several years before those doctors qualify, but by the end of the next parliament we will make the NHS self-sufficient in doctors,” Hunt said.

New opportunities for Australian doctors jobs

Remain campaigners warned in the run-up to the referendum that the NHS, which relies heavily on foreign staff, would be hit if EU workers could no longer travel freely to work in the UK. Data from the General Medical Council records show that 30,472 doctors come from the EU and other countries in the European Economic Area, while 71,139 were trained elsewhere in the world outside the UK. In total, it is estimated that one in four NHS doctors obtained their primary degree outside of the United Kingdom. This spells opportunity for Aussie doctors.

The NHS is facing doctor shortages in many areas of the country across a variety of specialties including A+E and Psychiatry. Although EU staff already in the NHS have been promised they will be allowed to stay, there is no guarantee this will happen when the reality of Brexit hits. Despite promises to open more medical degree places it will be at least ten years before that provides any relief to the NHS. In the meantime, in a post Brexit world we expect that the UK government will look to recruit heavily from Australia, New Zealand, Canada, India and South Africa and reduce the red tape imposed on doctors from several countries. As Australia and New Zealand are currently training far too many junior doctors, fresh opportunities abroad may just come in time to relieve our local capacity issues. Whilst the Brexit brings much uncertainty to the NHS and wider community it could provide an excellent opportunity for Australian’s to widen their career opportunities with new Australian doctors jobs in the UK.

What Drives Doctors mad when applying for Medical Jobs

For a country of just over 23 million people Australia has created a uniquely complex bureaucracy. The multi-level government structure is a major reason Australia is not the “nimble’’ country of innovation that our politicians claim they want us to be. The medical recruitment sector is littered with poor recruitment experiences for doctors who are just doing their best to secure suitable medical jobs. A number of their issues are related to this complex system that our health professionals are asked to navigate.

As the director of a medical recruitment firm specialising in the medical sector I regularly talk to outstanding doctors who have been left with a bitter taste in their mouth as a result of a hospital recruitment experience. For the most part I’m talking about direct applications to hospitals and practices for permanent medical posts rather than locum contracting jobs . Either way, finding the right medical jobs is just the first part of the challenge and the industry is falling behind in best practice.

I spoke with a senior medical recruiter from one of our largest health districts in Australia to understand the complaints that doctors have when trying to find medical jobs in Sydney or around Australia and how frustrating the recruitment process can be.

Common Complaints

Digital Voids:

The majority of hospitals in Australia utilise online recruitment platforms such as erecruit. Doctors have described these as “digital voids” because these platforms don’t keep applicants informed about the recruitment process. Responses or rejection letters can be sent out months after the closing date, if at all, and applicants have no way of telling if they are genuinely in the running for the post they have applied for. If there is one thing that applicants for any job, let alone emergency medical jobs want, it is information and communication.

Confusion and Duplication:

Frustrations in these recruitment platforms are compounded by doctors having to invest significant energy into the application process. Applicants may be asked for duplicate forms or to provide references when their application is confidential. Doctors may be reluctant to approach their referees when they don’t know where they are at in the process.

Position Descriptions:

Position descriptions in medical recruitment have become so generic as to be completely meaningless. They are simply “management speak’ and barely represent the actual role being applied for. As well as being baffling for the doctor trying to prepare their job applications they are viewed in a dim light by the Department of Immigration when assessing an individual’s 457 visa application. Employers in the private sector are moving from position descriptions to job profiles and briefs as they give a more realistic picture of what the actual job requires. It’s time medical recruiters follow suit.

Ensure that National Safety and Quality Health Service Standards are Provide governance and strategic direction for the development, implementation and evaluation of patient safety and quality improvement programs that promote the best possible experience and health outcomes for patients/clients. Timeframes – Preferred candidate or offer in principle

Another major source of agitation for doctors applying for medical jobs is timeframes. CareerArc tells us 60% of candidates say better communication throughout and after the hiring process would have the biggest impact on improving their experience. In the public sector timeframes can blow out, the recruitment process can stall or be completely abandoned altogether. Nobody seems to have control as the wheels of government turn slowly or grind to a bureaucratic halt. When this happens the first victim will be the restless applicants needing news on their application so they can plan their lives. The applicant may need to sell their house or move their families thousands of miles and they simply need to know where things are at in the recruitment process.

In this situation the hospital should employ a talented recruitment manager who can keep the communication channels open, manage the process and protect the reputation of the hospital. It is important to understand that these doctors go on to talk about the hospital. Their experience which could damage the brand and even reduce the quality of applicants that are attracted to the hospital over the long term. Medical staffing in hospitals has traditionally been a reactive department within the hospital, trying to manage an ever growing mountain of paperwork. As happened in the private sector, these departments must evolve to be more proactive and their aim should be to source the finest talent rather than simply processing applicants. Because these doctors become the human face of the hospital they are the ones who uphold its reputation. A good recruitment experience will make the best doctors want to engage with them and their hospital.

Smoothing out the recruitment process for medical jobs

A number of doctors identified problems with the recruitment process after they have been offered a medical job. Post candidate care can vary from delays with receiving an official offer to a lack of clarity regarding salaries and allowances. A typical response may be, “we don’t know what you will be paid until HR processes the contract.” This is simply unacceptable and HR departments must be clear about offers before contracts are drawn up. In addition candidates are kept in the dark regarding relocation packages. Rather than a short statement informing candidates of what and how much they can claim, the issue is often avoided. With the lack of a clear procedure nobody wants to make a commitment.

This lack of post candidate care can and does lead to candidates rejecting good medical jobs before they have even started. This leaves the candidate angered and costs the hospital or practice in wasted effort and time.

In order for hospitals to secure the very best medical talent they must utilise a combination of sophisticated IT platforms and human interaction. It takes more than technology to build the trust levels and explain the vision for their department or facility. Hospitals should hire or partner with the right recruiter who will ensure hospitals protect their reputation and get the best of medical talent over the long term. What It’s Like to Travel Locum Your Way Around Australia

Snorkeling the Great Barrier Reef. Running into a Cassowary in the Daintree Rainforest. Hiking to see Aboriginal rock art in Carnarvon Gorge. Squeezing through the dark passageways of Capricorn Caves. Walking around the base of Uluru. Looking out for Saltwater Crocodiles in Kakadu National Park. These are just a handful of the memories we have made during the past six months while doing travel locum work !

In January 2016 our family of ﬁve left our home in Brisbane to travel around Australia in a camper trailer for twelve months. In preparation for the new lifestyle, we sold most of our belongings and moved the rest into storage.

WHY WE DECIDED TO TRAVEL

Most people assume that we are travelling because we want to see the gorgeous country of Australia. While this is true, in part, the larger reason we decided to travel was to challenge ourselves to grow in new ways.

First of all, it was great timing for my husband’s medical career. He had recently passed his Emergency Medicine consultant exams after several years of study. If we ever wanted to take an extended period of time to travel.

We also knew that by doing travel locum work in different hospitals around the country, we would get a feel for what different communities would be like to live and work in. From the mining town of Mount Isa QLD to the large Aboriginal community in Katherine NT, he is getting a wide variety of clinical experience.

HOW WE FUNDED THE TRIP

We had not saved any funds for our trip, so we simply budget month to month and ﬁgure out how much we need to work along the way. In general we have taken a two to three week travel locum job every two months in order to cover our expenses. James from Medipeople simply sends us a list of available travel locum positions and we choose which jobs will coincide with our travel plans. Each travel locum job gives us a little break from the travelling lifestyle and helps us regain our momentum. I honestly do not think I would be able to travel 12 months full on had it not been for these “mini holidays” in hospital provided accommodation. Staying in a furnished house or unit is a welcome experience after 6 to 8 weeks of being on the road, and my husband enjoys the challenge of a frequently changing work environment.

FAMILY LIFE

Regarding the children, there are pros and cons to travelling while they are young. Our kids were 1, 5 and 7 when we started out. The beneﬁt of going early rather than later is that the kids were not already established in school like they would be when they are older. It does mean, however, that we have to set a pace that is reasonable for our little travellers and the types of activities we participate in are occasionally limited. We don’t usually drive more than 10 hours in a week, and one touristy type outing is usually enough for each day.

All in all, every family member is ﬁnding out what it means to truly value moments not things, and getting to see so much of this amazing country we live in is, in a word, priceless. I don’t think we will ever be able to explain how much we have all gained from this experience.

Yes, we have had a ﬂat tyre and other mechanical problems. Yes, we have had to set up camp in the pouring rain. Yes, we have had to sleep in 42 degree heat. Yes, we have had multiple screaming kids on a long car ride. But I wouldn’t change a thing!

TRAVEL LOCUM WORK

With all the fun memories and insights we have had during our trip, I can safely say this will always be one of our favourite years to look back on.

I would recommend, to anyone who can swing it, to deﬁnitely give travel locum work a go. You might regret not doing it, but you won’t ever regret doing it.

1. Set up boundaries and stick to them

2. Know your privacy settings

Do post and share. Being aware that what you say and how you say it, may be shared and made public, should influence your content considerably. Being mindful of what you say and share whether comments, photos or videos, will help you to maintain a credible, professional reputation.

Do Duty of Care. Mankind is your ticket, so treat everyone with the humility and professionalism you would if they were your only patient, colleague, contact or friend. Remember you can be googled by your present and/or future employers, colleagues, as well as family, friends and the general public for any reason at all. Don’ts

1. Professional vs Private Accounts

Don’t be naïve and think your personal, online account is your freeway to let your guard down. It is just as important to display and encourage appropriate behaviour as with your professional account. To maintain acceptable standards as a registered medical practitioner, student or trainee this must always be your guiding factor for untainted life/work relationships and reputation.

2. Say it only if you want to share it

Don’t be fooled by the delete button. Once you have posted something on social media, that’s it. Someone may have read it or shared it before you realised the implications and deleted it. It is recommended by site administrators that if you do not want certain information shared, do not put it online, cyberspace has an uncanny ability to store data for a very long period of time.

3. Confidentiality vs Confessions

Don’t breach patient confidentiality, no matter how much you really want to impress or impart that particular information online. The implications are far more reaching and debilitating than you think, with negative consequences impacting on your personal integrity as well as your professional status.

Working in Australia as a GP

Working in Australia as a General Practitioner (GP) is similar clinically to the UK. Most practices now are paperless so all patient records are accessed and recorded by doctors on computer. On average a practice will have 3 to 6 GP’s working full time and part time, with a practice nurse, treatment room, and some allied services such as Psychology, Physiotherapy and Pathology.

There are much larger practices that have up to 20 plus doctors and specialist consulting rooms on-site. They are generally owned by companies “corporates” or a group of doctors who have expanded over time. Solo GP practices still exist however mainly exclusive country and rural areas.

Billing and Earnings

When you’re working in Australia as a GP, you will find that some processes are working differently from e.g. the UK. The main difference between the UK GP practices and Australian GP practices is the way GP’s get paid. In Australia, practices can be private billing, bulk billing, or mixed billings. Private Billings means the practices charge its patients cash. Bulk billing is where the GP’s charge the patient through Medicare (Australia’s version of the NHS) for the consultation. Mixed billings are when a practice charges either private or private and bulk billings for the consultation. These mixed billing practices tend to “bulk bill” the elderly, concession holders, and children. The rest are privately billed. The money is then split between the practice and the GP as a percentage generally 65% of gross billings going to the GP.

This method has allowed GPs working in Australia to earn well over $250,000 (£150,000) per annum because of the income opportunity it presents. Even though the tax system is about 5% higher than the UK, Australian GP’s have a better tax advantage as they can leverage their income like a business (self-employed) and pay less tax at the end of the financial year. Eligibility

To be eligible to start working in Australia as a UK General Practitioner you must have the MRCGP (Membership of the Royal College of General Practitioners) holding the Certificate of Completion of Training (CCT) issued by the Post Graduate Medical Education and Training Board (PMETB) or the General Medical Council (GMC) or trained in Ireland and a graduate of the ICGP and hold MICGP.

Location Restrictions

The majority of overseas trained GP’s working in Australia are subject to District of Work Shortages (DWS) locations in order to be eligible for a provider number through Medicare. These areas are identified using the latest Medicare billing statistics, which are updated on an annual basis to account for changes in the composition and geographic distribution of the Australian medical workforce, and the latest residential population estimates as provided by the Australian Bureau of Statistics (ABS). DWS locations are mostly in outer metropolitan areas from about 30 minutes’ drive outside major cities.

Qualifications and Migration

The Practice Experience Program Specialist Stream provides a pathway for International Medical Graduates to start working in Australia based on an assessment of your previous training and experience in general practice.

The SPP visa pathway allows GPs to be living and working in Australia for up to 4 years via the TSS visa (dependant on the length of the job offer). It can be renewed multiple times, each time for up to four years, and allows dependents to be travelling, living and working in Australia with you for the same duration.

Eligibility for permanent residency in Australia is generally assessed on by taking into account a number of factors which can include:

Medical registration status
Type of job offer
Specific job demand
Relatives already living in Australia or NZ
Experience and/or qualifications

Each factor contributes a certain number of points and if you are able to satisfy the required amount then you may be eligible to apply.

Virtual Medicine

Essentially both concepts are referring to ‘virtual medicine’ where a patient can connect with a doctor virtually without having to go into a clinic for a checkup and being able to receive a diagnosis and in most cases a prescription online.

Telemedicine as it’s known can be most useful for simple things (for example repeat scripts) and basic care when it is inconvenient to visit a doctor in person, out of standard clinic hours or in a remote or rural community with limited doctor services available.

Issues with Telehealth

There are several key issues with Telemedicine.

The world is moving towards an integrated health model as best practice. Telemedicine is almost the opposite approach which will provide a fragmented and uncoordinated approach to healthcare.
Staff turnover could potentially be huge. A patient may never speak to the same doctor twice. As mentioned in the New York Times article the patient chose a doctor simply because of “a trustworthy face”. Many GPs will be reluctant to use this.
There are questions regarding software integration with other health practitioners’ notes and the patient’s medical history.
Older patients and those in rural areas may not have adequate IT skills to do this.
There is a massive danger of symptoms being missed due to a lack of physical examination – by feel, by smell, by empathy and the over-subscribing of antibiotics as a ‘quick fix’.

In cases of follow up care, long term care and where a doctor already knows a patient there are can definitely be merits in Telehealth and if managed correctly can actually enhance health care.

However at this point in time it is recommended that pro-telemedicine doctors and patients proceed with caution and should only try to conduct virtual health care within an established doctor/patient relationship where possible.

The phone interview is a crucial step in the medical recruitment process and the degree of preparation required will vary depending on the seniority of the position. We know it can be frustrating to get the job you want, so here are some interview tips to help you get the most out of your next phone interview!

Lock in the date

Due the time difference, the interview time and date are most likely to be early in the morning or late in the evening and not ideal for anybody! Rather than ask to rearrange another time make every effort to be available at the requested time. It is likely that the interview panel have set times and cannot easily be moved about. Asking to move the interview runs the risk of it not happening at all!

Research the location and hospital

You will have been provided with the standard job description and overview but why stop there. Check Google for more information about the area and hospital. Australia is a huge place and the hospital could be located in a town you have never heard of. Read up on the area. The panel will be impressed.

Research the phone interview panel

Thanks to Google and LinkedIN it is easy to find out information about our colleagues. If you can find information on the interview panel you many find common themes of clinical interest (or they may even be a fellow alumni). This is great preparation and will give you the chance to break the ice at the start of your phone interview.

Speak to your recruiter about likely interview questions and the interview format.

Provide two phone numbers

It sounds obvious but many a phone interview has been derailed by some kind of problem connecting to the number provided.

Prepare your interview space and technology

Decide prior to the phone interview where you will be based. Make sure there is minimal background noise and good phone reception. If you have a Bluetooth headset consider using it. Have a pad and pen to make notes of points or questions for later.

Manage your perception

You only get one chance to make a first impression. Greet the callers in a clear and friendly manner. Make sure you remember their names from your phone interview notes. Your clinical skills and experience are important but remember they are planning to hire a doctors from overseas. There is a lot of work required for hospitals to sponsor so they need your enthusiasm to shine through. Leave them in no doubt if you like the sound of the position that you will be prepared to accept the offer. Drink some water!

Ask relevant questions

Have one or two prepared questions you would like to ask them in your phone interview. Topics could include the job and hospital but also the local. If you have family you may want to know about the local schools or work opportunities for a spouse.
